Sign up Cost 9/23/2018 thru 10/14/2018 is $105 per player ($25 late fee applied after 10/14/2018)
Uniform is not included in sign up cost. COPY OF BIRTH CERTIFICATE required.
Personnel will be at the F.O.R. office located at the Owasso Sports Park, 10320 E 116 N. on
Sat Sep 29, Oct 6 & Oct 13 from 10am to 2pm. Sundays Sep 30, Oct 7 & Oct 14 from 2:00pm to 5:00pm.
Wednesday Oct 17 from 5:00pm to 7:00pm
Copy of birth certificate (Fax 918-371-9281). If you are a new Player, please come by the F.O.R. office if
you have any questions. FUNDRAISING AVAILABLE.
